The weather conditions were sunny at the time of
the flight and the upper portal areas had a significant amount of snow cover. All five portal areas, the lamp -house
and the office area were photographed. The permit for this mine was revoked and the bond was forfeited. The
reclamation was completed by the DRMS.
HYDROLOGIC BALANCE - Rule 4.05 Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5,
4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water
Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8;
Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
The two culverts in Coal Creek (below the Sutey Pile and at the confluence with Dutch Creek) appear to
be functioning properly and Coal Creek appears to be within the channel. The road over the culvert in Dutch
Creek by the lamp -house appears to have cracks (photo 1). This should be checked during the next ground
inspection.
The two ponds south of the Sutey Pile contain water and appear to be stable. The eastern pond has a large
sediment plume extending from the inlet channel.
ROADS — Rule 4.03 Construction 4.03.1(3)/4.03.2(3) Drainage 4.03.1(4)/4.03.2(4) Surfacing and
Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) Reclamation 4.03.1(7)/4.03.2(7):
A land slide was seen about .8 miles east of the office on the mine access road that has deposited some soil
on the road, but the road appears to be passable (photo 2). It is unclear from the maps if this area is within the
permit boundary.
Number of Partial Inspection this Fiscal Year: 1
Number of Complete Inspections this Fiscal Year: 1
Page 2 of 5
