Pineda <br />Director <br />The Division of Reclamation Mining and Safety (Division) received the Notice of Intent (NOI) to Conduct <br />Prospecting Operations for Hard Rock/Metal Mines for the Sunrise Lode, File No. P- 2012 -009 on May 30, 2012. <br />The Division has reviewed the NOI application for adequacy. Please address the following items: <br />Application: <br />2. Location – on page 2 of the application form are boxes to check for Quarter Section and Quarter /Quarter <br />Section. Please check the appropriate boxes to assist the Division in locating the prospect site. <br />3. Location Information – General Description on page 2 indicates the site is 3 4 mile from "Cripple ". Please <br />clarify that you mean "Cripple Creek" and in which direction from the site (i.e., north, east, west, etc.) <br />4. Location – page 3 of the application: Item 12.I and Item II – Maps & Drawings requires a USGS 7.5 <br />minute quad map (or similar) to provide an accurate topographic base map of the prospect area. Please <br />provide a topographic map showing the location of the Sunrise Lode, existing access roads, approximate <br />proposed trench locations, and approximate location of pre - existing drill holes described in Item 12.F on <br />page 4 of the application. <br />5. Project Description – Item 11I.8, page 3 indicates prospecting began in September, 2011. Please verify or <br />correct the date prospecting is expected to begin. <br />6. Project Description – Item III.11, page 4 indicates prospecting is limited to "trenching ". Please provide <br />some discussion as to the depth, width, and length of the proposed trenches. This information is needed to <br />estimate your financial warranty. <br />7. Project Description – Item II1.12.F Other Disturbances, page 4 indicates the existence of pre - existing drill <br />holes. Please provide approximate locations of existing holes for future reclamation purposes. <br />8. Project Description – Item I1I.12.F New Roads, page 4. Please provide some discussion on how site access <br />is obtained for the proposed backhoe /trackhoe equipment. Are there existing roads? Will the excavation of <br />trenches be limited to areas adjacent to existing roads? Please clarify your intentions with respect to <br />disturbance on and off any existing roads. <br />9. Operation and Reclamation Measures – Item IV.12.A, page 7. Please provide plant name and seeding rates <br />for reclamation.
