The bulkhead design report submitted with <br /> this revision states: <br /> The bulkhead designs contained in this report have been <br /> conservatively prepared. Conservatism was necessary because <br /> of the safety and environmental implications of a bulkhead <br /> failure, the long life required for the bulkheads and the <br /> ultimate inaccessibility of the bulkheads. <br /> Based on the mechanics and chemistry of concrete and grout <br /> degradation, as well as any case histories that may be applicable, <br /> what is the best estimate for the functional life-span of the <br /> proposed bulkheads? What, in descending order of probability of <br /> occurrence, are the likely failure modes for the bulkheads at the <br /> end of their functional life? What warning signs will there be <br /> that the bulkheads are failing? And finally, what would be the <br /> probable downstream consequences of the likely failure modes? <br /> If you are unable to prepare your responses to the foregoing <br /> concerns prior to the April 30, 1993 decision date for this <br /> revision, you will have to submit a written request for deadline <br /> extension. Failure to do so would result in revision denial. Once <br /> your responses are received by the Division, we would most likely <br /> set a new decision date to allow Division staff time to review the <br /> responses. On the decision date, whatever that date ultimately <br /> ends up being, the Division will either approve or deny the <br /> revision, or set the matter for a hearing before the Board. If the <br /> matter is set for a hearing, the Division will provide Sunnyside <br /> Gold with a synopsis of the recommendation Division staff will make <br /> to the Board and justifications for that recommendation. Be <br /> reminded that since this revision is open to public comment, <br /> additional concerns in addition to those listed in this letter, may <br /> still come up and need to be addressed by Sunnyside Gold. <br /> If you would like to set up a meeting to discuss issues related to <br /> your technical revision submittal and this list of adequacy <br /> concerns, or if you have any questions, please contact me. <br /> Sincere , <br /> Allen C.
