(Page 2) <br /> MINE ID # OR PROSPECTING ID # M-77-378 <br /> INSPECTION DATE 9-9-93 INSPECTOR'S INITIALS ACS <br /> OBSERVATIONS <br /> This inspection was conducted primarily to assess the progress of a program to collect water <br /> quality and quantity inventory data for drainages surrounding the mine. This program was <br /> initiated by Sunnyside Gold (SGC) in response to the Division's adequacy concerns related to <br /> the initial TR-014 submittal, specifically outlined in item #2 of Division's letter dated <br /> April 15, 1993. <br /> The information collected by SGC during the summer of 1993, and during future monitoring and <br /> sampling, will be supplemented by information collected for the Colorado Department of <br /> Health's Upper Animas Study, and information collected by SGC over the years in conformance <br /> with requirements of their reclamation and discharge permits. The purpose of this data <br /> collection and assembly, is to establish background hydrologic conditions that can be used <br /> to evaluate the performance of the proposed hydraulic bulkhead seals, and the impact of <br /> flooding the Sunnyside Mine workings. <br /> SGC used aerial photographs and USGS quads of the study area as a tool to develop the <br /> inventory plan, and to delineate the study area and document sampling locations. <br /> This inspection focussed on sample points along Cement Creek, the drainage identified by <br /> Simon Hydro-Search as the most probable receiving stream for groundwater flow from the area <br /> of the Sunnyside workings. The following sampling points were visited in the Cement Creek <br /> Basin: <br /> Mogul Mine #3 Level Mogul Mine #1 Level Swamp below the Mogul Mine <br /> #1 Level (Mogul Springs) <br /> Seep 4 Seep 13 Seep 9 <br /> Seep 5 Seep 6 Seep 7 <br /> Seep 13 is an adit that was dry at the time of the inspection, however, small watercourses <br /> at the portal indicate that it has flowed historically. The hillside below the Mogul Mine <br /> #3 Level dump was noted to be somewhat marshy. Seep 5 appears to be a true expression of <br /> emergent groundwater just above the level of Cement Creek. Seep 5 is flowing under pressure, <br /> iron-stained, and there are no surface water courses in the vicinity above the seep. <br /> The South Fork of Cement Creek was also visited as part of this inspection. The Sample point <br /> at the Silver Ledge Mine was inspected - this is also a sampling point for the CDH Upper <br /> Animas Study. Seepage was noted along the road cut leading to the Silver Ledge Mine. <br /> The Lead Carbonate Mine in Minnehaha Basin was visited - there was no flow from the mine at <br /> the time of this inspection. It was noted that the bottom of Minnehaha Basin is a swamp. <br /> Sunnyside Basin was also visited during this inspection. The operator had excavated some <br /> pits up-gradient from the Lake Emma area to demonstrate that groundwater flow is through <br /> undisturbed colluvium. It was noted that flow from the test pits was in undisturbed glacial <br /> till - it was also noted that the test pits were down hill from the mine open pits. <br /> The procedures observed during this inspection that the operator has initiated in order to <br /> characterize the existing hydrologic conditions in drainages surrounding the mine are <br /> adequate to satisfy adequacy concerns raised by the Division.
