DATE: March 14, 2001 <br /> TO: Wallace Erickson <br /> FROM: Allen Sorenson <br /> RE: Stipulations for Approval, Sunnyside Gold Corporation, Sunnyside Mine, <br /> Technical Revision TR-025,Permit No. M-1977-378 <br /> Stipulation No. 1 <br /> The degree of irregularity of the blasted surface of the tunnels through the reach of the proposed concrete <br /> bulkheads, after the scaling and washing operations, must be quantified and shown to be sufficient to prevent <br /> shear failure along the concrete-rock interface under the applied hydraulic thrust and in accordance with the <br /> design criteria for irregularity specified in the TR-025 application. <br /> Stipulation No. 2 <br /> The Division of Minerals and Geology will require that a copy of all documents related to technical revision TR- <br /> 025 be placed on permanent file(recorded) with the San Jaun County Clerk and Recorder and that Sunnyside <br /> Gold provide an affidaNit or receipt indicating the date on which the documents were placed with the county clerk <br /> and recorder. A permanent filing is deemed necessary in the case that any activity that may take place in the area <br /> in the distant future may require reference to the plans and designs for the bulkheads, and a filing with the county <br /> clerk is seen as the best way to ensure that the information will be available. <br /> Stipulation No. 3 <br /> American Tunnel Bulkhead No. 2 will be provided with a sampling port that will allow measurement of <br /> hydrostatic pressure behind the bulkhead and will allow samples of the impounded water to be collected <br /> for analysis. Bulkhead No. 2 will remain accessible for inspection and monitoring until it can be <br /> demonstrated that the bulkhead's performance is acceptable. The demonstration of acceptable <br /> performance must include, but may not be limited to: <br /> • Engineer's construction report and certification that the bulkhead was installed in accordance with <br /> the approved design and specifications. The construction report will include documentation of <br /> pertinent observations during construction, listing of the results of quality control and quality <br /> assurance tests, and a discussion of any problems encountered during construction and their <br />
