EA <br /> FAX:(303)832-8106 <br /> & <br /> GEOLOGY <br /> RECLAMATION <br /> DATE: December 13, 1999 1MINING-SAFETY <br /> Bill Owens <br /> TO: Larry Perino, SGC Reclamation Manager Governor <br /> Sarah Johnson, CDPHE WQCD Greg E Watcher <br /> Executive Director <br /> Michael Long <br /> FROM: Wally Erickson l�e � i���Sly9 Division Director <br /> RE: Project Completion Inspection, Surface Drainage Modification Project for <br /> Tailings Pond 4, CDPS Permit No. CO-0044768, Consent Decree and Order, <br /> Case 94 CV 5459, Sunnyside Mine, DMG Permit No. M-77-378. <br /> Pursuant to CDPHE WQCD request, dated November 15, 1999, DMG conducted a project <br /> completion inspection of the Surface Drainage Modification Project for Tailings Pond 4 at <br /> the Mayflower Facility. <br /> Inspection Date: November 16, 1999 <br /> SGC Representative: Larry Perino <br /> DMG Inspector: Wally Erickson <br /> Summary: Sediment containment ditch, located along the toe of the embankment <br /> outslope for Tailings Pond 4, and culvert outlet for the ditch, located under Highway 110, <br /> identified in SGC's report to Ms. Sarah Johnson, CDPHE WQCD, dated November 8, 1999 <br /> were the focus of this inspection. <br /> Installation of the drainage control structures appeared consistent with SGC's November 8, <br /> 1999 report, and appeared to have been completed in accordance with the conditions of <br /> the Consent Decree and in accordance with the performance requirements of the Colorado <br /> Mined Land Reclamation Act and the Hard Rock/Metal Mining Rules and Regulations of the <br /> Colorado Mined Land Reclamation Board. <br /> Observations: Construction of the sediment containment ditch, located along the toe <br /> of the embankment outslope for Tailings Pond 4 was complete. The sediment containment <br /> ditch conveys surface drainage from the slope of the pond embankment, and road drainage <br /> from Highway 110, to a culvert located under Highway 110.
