Any materials used during cleanup operations will be disposed of properly in a manner that is in
compliance with applicable EPA regulations.
5.4 Notification [112.4 and I I2.7(a)(4)]
After making the appropriate phone calls and the spill is contained, a "Spill Notification Form,"
included in Appendix H, shall be completed and submitted to the Environmental Compliance
Officer. The "Spill Notification Form" includes a checklist to document the proper notification
of state and federal agencies. The form shall be filed by facility name and maintained as long as
NECC owns and/or operates this facility.
If a single spill greater than 1,000 gallons occurs, or two spills greater than 42 gallons each occur
within any 12 -month period at this NECC facility, the Environmental Compliance Officer shall,
follow the notification procedures below and provide written information to the EPA Regional
Administrator as required by the federal SPCC rules. A copy of this information must be
provided to the Colorado Department of Environmental Protection (Appendix I).

• Date & time of the discharge
• Estimated size and what type of material was discharged
• Any corrective actions taken to abate discharge
• Possible cause of discharge
• Any additional measures taken
• Additional pertinent information
