MAY -16 -2012 WED 03 11 PM HR SAFETY L44.e4 �-ke FAX N0, 3037165299 <br />1�73 0,9 <br />May 16, 2012 <br />Colorado Division of Reclamation, Mining and Safety 1 <br />The Centennial Building <br />1313 Sherman St., Rm. 215 <br />Denver, CO 80203 <br />Attention: Tom Kaldenbach <br />Re: Aggregate Industries' Morrison Quarry -400 Gallon Dlesel Spill <br />Dear Tom, <br />P. 01 <br />Ag& <br />AGGREGATE <br />INDUSTRIES <br />On May 14"', 2012 at approximately 9:00am a fuel truck fueling a 980 Cat Loader blew a gasket and <br />spilled approximately 400 gallons of diesel fuel (CAS # 68476 -34 -6) on the quarry floor of the Morrison <br />Quarry (M- 1973 -021) located at 18131 Colorado Hwy 8 in Morrison, CO. <br />The spilled product was contained within an approximately 700 sq foot perimeter, as the loader was <br />being fueled in a slight depression. The entirety of the spilled product was soaked up by gravel and <br />crushed gravel material on the quarry floor and the area bermed with stripping material until further <br />mitigation practices can take place. <br />Aggregate Industries is currently working with Jerry Henderson (303 - 692 -3455) of the Colorado <br />Department of Public Health and Environment on a mitigation plan which at this point appears to <br />moving the contaminated material to an approved landfill. <br />The spill was reported by myself (Doug Hess) to CDPHE's Spill Response line (877 - 518 -5608) at <br />approximately 1 -30pm on Monday the 14`h and recorded by Cory Porter (303 - 692 - 2967). Additionally, <br />the spill was reported to the State Oil Inspector (303- 318 -5847) later that afternoon. <br />If you have any questions or concerns, please contact myself or Kipp Raines, the Quarry Manager at 720- <br />308 -4188.
