Compliance With Mine Plan N - Support Facilities Not On -site <br />R - Other N - Special Categories Of Mining <br />N - Processing Waste N - Topsoil <br />COMMENTS <br />This was a partial inspection conducted by Janet Binns and Kent Gorham of CDRMS. Mr. George Patterson of <br />Energy Fuels Coal, Inc. met the inspectors at the MW -NW about 12:30 pm. The weather was warm and clear with <br />a steady breeze. George infonned the inspectors that the area had received rain 3 days prior, on 5/12/2012. The <br />ground surface had damp spots and was soft. <br />The focus of this inspection was to observe the condition of Water Monitoring Well MW -NW using the Division's <br />borehole camera. The inspection also focused on observations of the previously identified subsidence holes on Dr. <br />Corley's property located east of the Thompson ranch house. <br />HYDROLOGIC BALANCE -Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3, 4.05.6; ; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br />4.05.13; Stream Buffer Zones 4.05.18: <br />On April 17, 2012 the Division had inspected Mine water monitoring well MW -NW. At that time the <br />operator had pumped approximately 4000 gallons of water (> 3 calculated borehole volumes) into the hole to <br />determine if the well had communication with the mine and hence was accurately measuring the water elevation in <br />the mine. <br />Energy Fuels' records indicate that well MW -NW was completed to 380' below the ground surface and is open to <br />the mine void in order to measure mine water elevation. On April 17, 2012, the Division measured water at 354' <br />below the surface is the MW -NW well, after the water had been pumped down the well. Energy Fuels has been <br />recording MW -NW as dry to 380' since monitoring of this point began in 2003. <br />The down -hole camera was employed to investigate the condition of the well at 354' and 380' below the surface. <br />Kent Gorham had previously been trained on operation of the borehole camera. The camera is a GeoVision Jr. <br />(tm) Borehole Video System.
