Thompson, <br /> -Mid-Continent Resources hereby requests a minor revision to <br /> Permit #C-017-81 in order to allow an addition to be built onto <br /> our #4 Fan located at #2 Mine. <br /> In the past few months we have recognized the need for more <br /> ventilation in the M-Bed. After reviewing several options it was <br /> decided that the best option would be to drive a new entry out <br /> near our #4 fan and build a concrete and steel connection to the <br /> existing fan ( figure 1 ) . <br /> The new portal area as shown on map 1 will occupy an area <br /> approximately 40 ' by 25 ' or . 02 acres . Much of this area has <br /> already been disturbed when the original #4 Fan was constructed. <br /> It will be necessary for us to remove the existing stockpile of <br /> dirt near the fan and new material at the face-up area. This <br /> small amount of material will be used to shore up the berms <br /> around the fan site. As I pointed out to you during the last <br /> inspection there is no available top soil at this site. <br /> Because the area of disturbance is very small we are asking for a <br /> small area exemption at this location . <br /> The bonding calculations for this site is as follows : <br /> Portal Sealing = $500 . 00 <br /> Structure Removal-3360 cu. ft . x $ . 13/cu. ft . = $436. 80 <br /> Final Grading - $70 . 00/ac . x . 02 ac. = $ 1 . 40 <br /> Topsoil - . 5 ' x . 02ac. x 43560 x $ . 85/yd = $ 15 . 30 <br /> . 9( swell factor ) x 27 <br /> Revegetation - $1395/acre x . 02 acre = $ 27 . 90 <br /> Sub-Total = $981 . 40 <br /> Indirect Cost @ 200 = $196. 28 <br /> Total Bond Cost =$1177 . 68 <br /> This minor revision request should be placed in Volume II , <br /> Appendix TI-11-6 , :'aife 7 in our permit application . <br />
