May 9, 2012 C- 1981 - 033 /13ear Mine <br />CIVIC <br />SLIDES and DAMAGE - Rule 4.12: <br />The slide area appeared to be stable and did not move or shift since the last inspection. No smoke was <br />seen coming out of the scarp during this inspection. A M40 Multi Gas Monitor was used to detect the presence of <br />mine gasses in the slide area. Cracks in the ground, rodent holes and voids around rocks were checked and no <br />mine gasses were detected. The areas where smoke was seen during previous inspections were not monitored for <br />mine gasses due to the dangerous conditions of these areas. <br />SIGNS AND MARKERS — Rule 4.02: <br />The mine identification sign was visible at the mine entrance with all the required information. <br />DOCUMENTS RECEIVED: None <br />ENFORCEMENT ACTIONS /COMPLIANCE <br />No enforcement actions were taken as a result of this inspection. <br />PHOTOGRAPHS <br />Photo 1 — Thistle Patch Where the Office Building Once Stood <br />Number of Partial Inspection this Fiscal Year: 7 <br />Number of Complete Inspections this Fiscal Year: 4 <br />Page 3 of 5 <br />
