It concerns <br /> the placement of hay bales around the topsoil pile as an interim <br /> sediment control measure. On completion of the pile, permanent <br /> sediment control measures were installed. No sediment loss <br /> occurred. <br /> CO C-89-019 (29-Jun-89) <br /> Cessation Order C-89-019 resulted because of an accident <br /> beyond the control of Mid-Continent at Outfall No. 016. During <br /> pond cleaning, a necessary maintenance requirement, an independent <br /> contractor's dragline operator struck and damaged the spill-pipe of <br /> the uppermost pond with the dragline's bucket. The dragline <br /> operator failed to report the damage. The damage caused the pond <br /> system to short-circuit and malfunction, and it resulted in <br /> sediment discharges to the Crystal River. <br /> NoV C-90-001 (04-Jan-90) <br /> NoV C-90-001 was written because of the operator's inability <br /> to get a full sample at groundwater well GW-1, in the Sutey coal <br /> refuse pile. No environmental damage occurred. <br /> NoV C-90-002 (04-Jan-90) <br /> NoV C-90-002 resulted from inadvertent oversight of an area <br /> of erosion caused by a major storm event for which a large-scale <br /> maintenance and clean-up program was carried out over the whole of <br /> the permit area. The cited area is on a hillside not readily <br /> visible from the road. Sediment from the area is controlled by the <br /> Outfall No. 001 pond series.
