EROSION CONTROL NOTES <br />1. THE OWNER, SITE DEVELOPER, CONTRACTOR AND /OR THEIR AUTHORIZED AGENTS SHALL <br />REMOVE ALL SEDIMENT, MUD, CONSTRUCTION DEBRIS, OR OTHER POTENTIAL POLLUTANTS THAT <br />MAY HAVE BEEN DISCHARGED TO OR, ACCUMULATE IN, THE FLOWLINES, STORM DRAINAGE <br />APPURTENANCES, AND PUBLIC RIGHTS OF WAYS AS A RESULT OF CONSTRUCTION ACTIVITIES <br />ASSOCIATED WITH THIS CONSTRUCTION PROJECT. SAID REMOVAL SHALL BE CONDUCTED IN A <br />TIMELY MANNER. <br />2. UNDERSTAND THAT ADDITIONAL EROSION AND SEDIMENT CONTROL MEASURES MAY BE REQUIRED <br />OF THE OWNER AND HIS OR HER AGENTS DUE TO UNFORESEEN EROSION PROBLEMS OR IF THE <br />SUBMITTED PLAN DOES NOT FUNCTION AS INTENDED. THE REQUIREMENTS OF THIS PLAN <br />SHALL BE THE OBLIGATION OF THE LAND OWNER AND /OR HIS SUCCESSORS OR HEIRS; UNTIL <br />SUCH TIME AS THE PLAN IS PROPERLY COMPLETED, MODIFIED, OR VOIDED. <br />3. THE CONTRACTOR SHALL PREVENT SEDIMENT, DEBRIS AND ALL OTHER POLLUTANTS FROM <br />ENTERING THE STORM SEWER SYSTEM DURING ALL DEMOLITION, EXCAVATION, TRENCHING, <br />BORING, GRADING, OR OTHER CONSTRUCTION OPERATIONS THAT ARE PART OF THIS PROJECT. <br />THE CONTRACTOR SHALL BE HELD RESPONSIBLE FOR REMEDIATION OF ANY ADVERSE IMPACTS <br />TO ADJACENT WATERWAYS, WETLANDS, ETC., RESULTING FROM WORK DONE AS PART OF THIS <br />PROJECT. <br />4. A LAYER OF SUITABLE MULCH SHALL BE APPLIED TO ALL DISTURBED PORTIONS OF THE SITE <br />WITHIN FOURTEEN (14) DAYS OF THE COMPLETION OF GRADING. SAID MULCH SHALL BE <br />APPLIED AT A RATE OF TWO (2) TONS PER ACRE AND SHALL BE TACKED OR FASTENED BY <br />AN APPROVED METHOD SUITABLE FOR THE TYPE OF MULCH USED. ROUGH -CUT ROADS SHALL <br />BE MULCHED UNLESS A LAYER OF AGGREGATE ROAD BASE IS TO BE APPLIED TO ROUGH -CUT <br />ROADS WITHIN THE 14 DAY PERIOD AFTER COMPLETION OF GRADING. <br />5. THE CONTRACTOR SHALL LOCATE, INSTALL, AND MAINTAIN ALL EROSION CONTROL AND WATER <br />QUALITY BEST MANAGEMENT PRACTICES AS INDICATED ON SHEET C4.20 AND C4.21, EROSION <br />CONTROL PLANS. <br />6. THE GENERAL CONTRACTOR, GRADING CONTRACTOR AND /OR THEIR AUTHORIZED AGENTS SHALL <br />INSURE THAT ALL LOADS OF CUT AND FILL MATERIAL IMPORTED TO OR EXPORTED FROM THIS <br />SITE SHALL BE PROPERLY COVERED TO PREVENT LOSS OF THE MATERIAL DURING TRANSPORT <br />ON PUBLIC RIGHTS OF WAYS. <br />7. THE USE OF STEEL STAKES OR STEEL FENCE POSTS TO STAKE DOWN STRAW WATTLES OR TO <br />SUPPORT SILT FENCING USED AS AN EROSION CONTROL MEASURE IS PROHIBITED. <br />8. SOILS THAT WILL BE STOCKPILED FOR MORE THAN THIRTY (30) DAYS SHALL BE PROTECTED <br />FROM WIND AND WATER EROSION WITHIN FOURTEEN (14) DAYS OF STOCKPILE CONSTRUCTION. <br />STABILIZATION AND PROTECTION OF THE STOCKPILE MAY BE ACCOMPLISHED BY ANY OF THE <br />FOLLOWING: MULCHING, TEMPORARY /PERMANENT REVEGITATION OPERATIONS, CHEMICAL SOIL <br />STABILIZER APPLICATION(REQUIRES WMD APPROVAL), OR EROSION CONTROL <br />MATTING /GEOTEXTILES. IF STOCKPILES ARE LOCATED WITHIN 100 FEET OF A DRAINAGEWAY, <br />ADDITIONAL SEDIMENT CONTROLS SUCH AS TEMPORARY DIKES OR SILT FENCE SHALL BE <br />REQUIRED. <br />9. APPROVED EROSION AND SEDIMENT CONTROL BEST MANAGEMENT PRACTICES SHALL BE <br />MAINTAINED AND KEPT IN GOOD REPAIR FOR THE DURATION OF THIS PROJECT. AT A MINIMUM, <br />THE CONTRACTOR OR HIS AGENT SHALL PRODUCE AND RETAIN BI- WEEKLY WRITTEN INSPECTION <br />RECORDS FOR ALL BMPs AND AFTER SIGNIFICANT PRECIPITATION EVENTS. ALL NECESSARY <br />MAINTENANCE AND REPAIR SHALL BE COMPLETED IN A TIMELY MANNER BUT IN NO CASE MORE <br />THAN SEVEN(7) CALENDAR DAYS AFTER THE INSPECTION. HOWEVER, STREET SWEEPING IS TO <br />BE COMPLETED BY THE CLOSE OF BUSINESS DAY OR ON AN AS NEEDED BASIS. <br />ACCUMULATED SEDIMENT AND DEBRIS SHALL BE REMOVED FROM A BMP WHEN THE SEDIMENT <br />LEVEL REACHES ONE THIRD THE HEIGHT OF THE BMP OR, AT ANY TIME THAT SEDIMENT OR <br />DEBRIS ADVERSELY IMPACTS THE FUNCTIONING OF THE BMP. <br />10. MODIFICATION OF AN ACTIVE CONSTRUCTION ACTIVITIES STORMWATER DISCHARGE PERMIT BY <br />THE CONTRACTOR OR THEIR AUTHORIZED AGENTS SHALL REQUIRE TIMELY NOTIFICATION. <br />DISCHARGE PERMITS EXPIRE IF CONSTRUCTION HAS NOT COMMENCED WITHIN 12 MONTHS OF <br />THE ACCEPTANCE OF THE MANAGEMENT PLAN. <br />GRADING NOTES <br />1. EXISTING DRAINAGE STRUCTURES TO BE INSPECTED AND REPAIRED AS NEEDED, AND EXISTING <br />PIPES TO BE CLEANED OUT TO REMOVE ALL SILT AND DEBRIS. <br />2. EXISTING GRADE CONTOUR INTERVALS SHOWN AT 2 FOOT. <br />3. PROPOSED GRADE CONTOUR INTERVALS SHOWN AT 2 FOOT. <br />4. IF ANY EXISTING STRUCTURES TO REMAIN ARE DAMAGED DURING CONSTRUCTION IT SHALL BE <br />THE CONTRACTORS RESPONSIBILITY TO REPAIR AND /OR REPLACE THE EXISTING STRUCTURE AS <br />NECESSARY TO RETURN IT TO EXISTING CONDITIONS OR BETTER. <br />5. EXISTING TOPOGRAPHIC INFORMATION SHOWN IS FROM A AERIAL TOPOGRAPHIC SURVEY <br />PREPARED BY LAND SURVEYORS. IF THE CONTRACTOR DOES NOT ACCEPT EXISTING <br />TOPOGRAPHY AS SHOWN ON THE PLANS, WITHOUT EXCEPTION, THEN THE CONTRACTOR <br />SHALL SUPPLY, AT THEIR EXPENSE, A TOPOGRAPHIC SURVEY BY A REGISTERED LAND <br />SURVEYOR TO THE OWNER FOR REVIEW. <br />6. CONSTRUCTION SHALL COMPLY WITH ALL APPLICABLE GOVERNING CODES AND BE CONSTRUCTED <br />TO SAME. <br />7. UNLESS OTHERWISE SHOWN, NO PROPOSED SLOPE SHALL EXCEED ONE (1.0) HORIZONTAL TO <br />ONE (1) VERTICAL. ALL SLOPED AREAS MUST BE PROTECTED FROM EROSION. <br />8. THE CONTRACTOR IS RESPONSIBLE FOR CALCULATING HIS OWN ESTIMATE OF EARTHWORK <br />QUANTITIES. <br />9. ALL EARTH MOVING AND PLACEMENT OPERATIONS SHALL BE IN CONFORMANCE WITH THE <br />RECOMMENDATIONS IDENTIFIED IN THE RULE 6.5 GEOTECHNICAL STABILITY REPORT PREPARED <br />BY J. ERICH RAUBER AND DAVID McLAY, DATED APRIL 2, 2012 FOR THIS PROJECT. THE <br />CONTRACTOR SHALL HAVE A SIGNED AND SEALED COPY OF SAID REPORT ON THE SITE AT ALL <br />TIMES. <br />10. THE CONTRACTOR SHALL ADHERE TO ALL TERMS AND CONDITIONS AS OUTLINED IN THE <br />GENERAL PERMIT FOR STORM WATER DISCHARGE ASSOCIATED WITH CONSTRUCTION ACTIVITIES. <br />STORM SEWER NOTES <br />1. STORM SEWER PIPE SHALL BE CORRIGATED METAL PIPE (CMP), AS SHOWN ON THE PLAN. ANY <br />OTHER PIPE SUBSTITUTIONS WILL REQUIRE PRIOR APPROVAL OF THE ENGINEER. <br />2. LENGTHS OF PIPE SHOWN ARE FROM CENTERLINE TO CENTERLINE OF MANHOLES OR INLETS. <br />THEREFORE, LENGTHS SHOWN ARE APPROXIMATE. <br />3. INLETS AND MANHOLES SHALL BE IN ACCORDANCE WITH THE DETAIL DRAWINGS. <br />4. CONTRACTOR 'SHALL VERIFY HORIZONTAL AND VERTICAL LOCATIONS OF PROPOSED STORM SEWER <br />POINTS OF CONNECTIONS PRIOR TO CONSTRUCTION OF ANY PROPOSED STORM SEWER. <br />5. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE FOLLOWING: <br />(A) OBTAINING; ALL REQUIRED PERMITS FROM THE COUNTY OR REGULATORY AUTHORITIES AT THE <br />CONTRACTOR'S COST INCLUDING PERMITS REQUIRED FOR WORK WITHIN THE PUBLIC RIGHT -OF -WAY, <br />(B) <br />RESTORATION <br />OF <br />ANY EXISTING <br />IMPROVEMENTS INCLUDING <br />(BUT NOT LIMITED <br />TO) <br />FENCES. <br />(C) <br />VERIFICATIION <br />AND <br />PROTECTION <br />OF ALL EXISTING UTILITIES <br />WITHIN THE LIMITS <br />OF <br />CONSTRUCTION <br />(D) PROVIDING; STORM SEWER AS- BUILTS TO THE ENGINEER INCLUDING DIMENSIONS, PIPE SIZES, LINES, <br />AND GRADE OF ALL DRAINAGE APPURTENANCES; INCLUDING BUT NOT LIMITED TO, MANHOLES, <br />JUNCTIONS, HEADWALLS, AND DRAINS. <br />6. RIM ELEVATIONS SHOWN ARE APPROXIMATE ONLY AND ARE NOT TO BE TAKEN AS FINAL ELEVATION. <br />PIPELINE CONTRACTOR SHALL USE PRE CAST CONCRETE ADJUSTMENT RINGS, GROUT, AND STEEL <br />SHIMS TO ADJUST THE MANHOLE FRAME TO THE REQUIRED FINAL GRADE. <br />7. COMPACTION OF ALL TRENCHES WITHIN THE PROJECT SITE MUST BE ATTAINED AND COMPACTION <br />RESULTS SUBMITTED TO THE ENGINEER PRIOR TO FINAL ACCEPTANCE. COMPACTION OF ALL TRENCHES <br />WITHIN THE PROJECT SHALL BE IN ACCORDANCE WITH AND IN COMPLIANCE WITH THE RULE 6.5 <br />GEOTECHNICAL STABILITY REPORT. <br />8. ALL STORM PIPE ENTERING STRUCTURES SHALL BE GROUTED TO ASSURE CONNECTION AT STRUCTURE <br />IS WATERTIGHT. <br />9.
