OBSERVATIONS <br />PERMIT #: M- 1981 -044 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: March 14, 2012 <br />1) The Division conducted a monitoring inspection of the site on 3/14/12. The site was active at the time of the <br />inspection. Present during the inspection was Mr. Bob Paython. <br />2) The entire permit and affected area was walked and inspected. When the site was amended in 2007 by adding <br />140.66 acres to the permit, for a total of 235 acres, the maximum affected acres were increased to 42 acres. The <br />current financial warranty for the site is $72,635.00. The warranty appeared sufficient to cover the cost of <br />current affected area. The site is being operated by Pioneer Sand for the permit holder. <br />3) According to the annual report and what was observed on site approximately 25 acres appeared to be currently <br />affected. The site consists of a series of settling ponds. There was what appeared to be a small fresh water pond <br />the was noted along the east side of the active mining area. The processed a material was being washed at the <br />time of the inspection. There was no exposed ground water that was noted at the time of the inspection. <br />4) A small what appeared to topsoil /growth medium was noted along the access to the property. It appeared <br />stable and vegetated. There were several large stockpiles of processed material noted along the West and south <br />side of the active mining area. The pit high wall orienting North/ South was stable and no major failure surface <br />was observed at the time of the inspection. <br />5) Permit sign and boundary markers properly posted. <br />Page 2 of 4 <br />
