OBSERVATIONS <br />PERMIT #: M- 1991 -147 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: April 10, 2012 <br />1) The Division conducted a monitoring inspection of the site on 4/10/12. Present during the inspection were Mr. Bill <br />Johannsen and Mr. Brad general manger of the quarry. The site was not active at the time of the inspection. <br />2) The site is a small limestone quarry located approximately 10 miles North West of Woodland Park. The site was mined pre <br />law. The current operator transferred the permit from Stone Forge in 2005. The material mined is used as either decorative <br />rock or building material. No crushing takes place on site. The site is only active on as need basis. <br />3) The post mining land use is residential once mining is completed. During the August 2007 inspection no problem was cited <br />and the warranty was recalculated and found to be adequate. Since that time very little has changed on site. The only <br />structures on site are a small screen a portable toilet as noted in the 2007 inspection. The current financial warranty for the <br />site is $5,000.00. <br />4) In order to avoid confusion as to the exact location of the permit boundary, it will be advantageous to the operator to have <br />the site surveyed. Even though the permit for the site was issued in 1992, this area has been mined pre -law. Because of the <br />lack of good map the Division was not able to clearly identify the current boundary markers. It will be advantageous to both <br />the Division and the operator to clearly define the permit boundary. Under a 110 permit the permit boundary and the <br />affected boundary are the same. The fact that the Division couldn't clearly identify the permit boundary will not be cited as <br />a problem at this time because it was identified clearly in 2007 inspection. With old sites, such as this one unless the <br />operator clearly delineate the the boundaries all of the pre- law disturbance could be assumed to be part of the permit and <br />thus will be governed by what is approved in the reclamation plan for the permit. Even though the post mining lad use is <br />residential and might not require top soiling and seeding, the site will still need to be graded to accommodate such a <br />structure. At this time the Division requests the operator have the area properly surveyed and place appropriate boundary <br />markers such as metal stakes and provide the new map to the Division no later than 90 days from receipt of this inspection <br />report. <br />5) The operation appears to be in compliance with the approved permit. No formed channels were noted leaving the affected <br />area into the dry gulch along the south side of the permit boundary. <br />Page 2 of 4 <br />
