OBSERVATIONS <br />PERMIT #: M -1989 -092 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: April 10, 2012 <br />1) The Division conducted a monitoring inspection of the site on 4/10 12. Present during the inspection was Mr. David Curtis. <br />The site was not active at the time of the inspection. The site is located approximately 3 miles south of Sadlia. <br />2) A permit sign was posted at the access to the pit. The total permitted acres is 25.69 with maximum affected acres of 10. The <br />site was converted from a 110 into a 112 on 7/7/2008 <br />3) There was a wood saw located along the south side of the pit, with wood logs stacked in the pit floor. The pit high walls <br />were stable with an approximate average slope of 4:1. The reclaimed slopes along the south west side appeared stable and <br />vegetated. The permit boundary markers were properly posted. The topsoil stockpile located along the north side appeared <br />stable. When demand picks up, according to Mr. Curtis active mining will be along the North side of the current pit area. <br />4) The current affected acres didn't appear to have increased since the site was last inspected in 2006. The total affected acres <br />appeared to be approximately 7 acres. The operator is aware that until the reclaimed acres are released in accordance with <br />Rule 4.17, those acres are still considered disturbed. There was no noxious weed infestation in the reclaimed slopes. The <br />operator has in place a noxious weed control plan. There was what appeared to be an explosive magazine located along the <br />east side of the pit area. The magazine was properly secured according to Mr. Curtis and he also stated he has the proper <br />permits from MSHA to store them on site. <br />5) The current financial warranty in the amount of $12,000.00 is adequate to cover the cost of current affected area. Mr. <br />Curtis stated that because of Douglas County regulations the high wall along the East side has to cover the pit area so it will <br />not be visible from the road. <br />Page 2 of 4 <br />
