Review of the information submitted by the Applicant determined the <br />following items must be received before the Division can consider the application as being <br />submitted and technical review can begin: <br />Application Form: <br />1. On Page 2, the Applicant did not provide quarter section and quarter /quarter section <br />information and provided two section numbers for Item #9 on the application form. Based on <br />Exhibit A — Legal Description and Exhibit B - Survey Map submitted in the application package, <br />the Division believes the proper location information is; Section 30, Quarter Section "SE" and <br />Quarter /Quarter Section "SW ". <br />Please review the location information for the center of the area where the majority of <br />mining will occur and revise the application form accordingly. <br />Exhibit R - Proof of Filing with County Clerk (Rule 6.4.18): <br />2. Please provide an affidavit or receipt indicating the date on which the revised application <br />form information indicated above was placed with the local County Clerk and Recorder. <br />The application will not be considered submitted until the information listed above is received <br />and found sufficient to begin our review. A decision date will be established 90 days from the <br />date of receipt of all of the requested information. Additionally, if you have already published <br />notice you will need to republish notice, after the Division considers the application submitted. <br />This notice must be published once a week for four (4) consecutive weeks, starting within ten (10) <br />days of the date your application is considered submitted. We will notify you when you should <br />initiate republication of your notice.
