If the determination is "may affect" for <br /> listed species, the Federal agency must request in writing formal <br /> consultation from the Field Supervisor, U.S. Fish and Wildlife Service, <br /> Fish and Wildlife Enhancement, 2078 Administration Building, 17,15) West 1700 <br /> South, Salt Lake City, Utah 84104. At this time, this agency should provide <br /> this office a hiological Assessment and/or any other re-Levant information <br /> used in making the determinations. <br /> The U.S. fish and Wildlife Service can enter into formal Section 7 <br /> consultation only with another Federal agency.
