Pineda <br />Director <br />On March 1, 2012, the Division of Reclamation, Mining and Safety received your transfer of Permit and Succession <br />of Operations Application Form for transfer from Hilltop Aquatic Investments to Alpine Demolition, Inc. <br />Per the terms of your permit you are required to submit an annual fee and report each year prior to your <br />anniversary date of March 4. The Division must receive all fees due accompanied by an adequate annual report <br />by no later than 5 p.m. on May 2, 2012 or a Notice of Violation, Cease and Desist Order from all mining related <br />activities, Civil Penalties equal to the amount of annual fee due $791.00, and a Notice of Formal Public Hearing <br />before the Mined Land Reclamation Board for consideration of POSSIBLE PERMIT REVOCATION AND <br />FINANCIAL WARRANTY FORFEITURE will be issued for this account. <br />The Succession of Operator Transfer cannot be completed until the annual fees and report are received. <br />Any question concerning your annual report or fee may be directed to Christine Brookshire at (303) 866 -3567 ext <br />8132 or Jim Dupler (303) 866 -3567 ext 8119.
