PART I <br /> Page 4 of 19 <br /> D. REPORTING <br /> I. Signatory Requirements <br /> All reports required for submittal shall be signed and certified for <br /> accuracy by the permittee in accord with the following criteria: <br /> a) In the case of corporations, by a principal executive officer of at <br /> least the level of vice-president or his or her duly authorized <br /> representative, if such representative is responsible for the <br /> overall operation of the facility from which the discharge <br /> described in the form originates; <br /> b) In the case of a partnership, by a general partner; <br /> c) In the case of a sole proprietorship, by the proprietor; <br /> d) In the case of a municipal, state, or other public facility, by <br /> either a principal executive officer, ranking elected official, or <br /> other duly authorized employee. <br /> Within sixty (60) days of the effective date of this permit, the <br /> permittee shall file a statement with the Environmental Protection <br /> Agency and the Water Quality Control Division which shall contain <br /> the names of the person or persons who are designated to report <br /> conditions on discharge monitoring reports and as noted in <br /> "Noncompliance Notification", Part II A. The permittee shall <br /> continually update this list as changes occur at the facility. <br /> 2. Data Reporting <br /> Part I.B.1 specifies the frequency for reporting data and the permittee <br /> shall submit data in accordance with the appropriate frequency below. <br /> a) Monthly <br /> Monitoring results shall be summarized for each month and reported <br /> on applicable discharge monitoring report forms received at this <br /> office no later than the 28th day of the following month. If no <br /> discharge occurs during the reporting period, "No Discharge" shall <br /> be reported. <br /> Code: 1 - 18 Date: 1 - 84 <br /> 7-6-12 <br />
