Pineda <br />Director <br />Enclosed you will find the Division of Reclamation, Mining and Safety's Decision Form for <br />Minor Revision No. 99 at the Elk Creek Mine. <br />The purpose of MR -99 is to provide for an additional four (4) angle boreholes on three (3) <br />existing GVB pads. The boreholes are designated GVB- LW- 17 -05B, 17 -06D, 17 -08D and 17- <br />08E. The 17 -05B borehole is on an existing pad located on private surface and the other 3 <br />boreholes are on existing pads located on USFS surface. No new road construction or <br />additional surface disturbance will occur. A list of conditions necessary to protect federal <br />resources, as set forth by the USFS, has been incorporated into Exhibit 2.03 -E1 of the permit <br />application package. Please continue to comply with all regulations set by the USFS. <br />Documentation must be submitted to the Division demonstrating that a copy of the approved <br />revision application was sent to the Office of Surface Mining and the Bureau of Land <br />Management. The material should be sent within 15 days of final approval. <br />Please refer to the Performance Bond heading on the enclosed decision form.
