OBSERVATIONS <br />PERMIT #: M- 2001 -107 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: April 10, 2012 <br />The NCCI Pit #1, File No. M- 2001 -107 was inspected by Peter Hays with the Division of Reclamation, Mining <br />and Safety (Division) in response to a technical revision submitted by Northern Colorado Constructors, Inc. <br />(NCCI). Mr. Chris Zadel and other representatives from NCCI were present during the inspection. <br />On March 12, 2012, NCCI submitted a Technical Revision (TR -07) proposing to re- establish the standard <br />deviation minimum and maximum monitoring well levels for monitoring Well No. 9 and No. 10. The scope of <br />this inspection was limited to areas of the mine site related to monitoring well compliance. The recharge ditch <br />along the south and southwest sides of the site were inspected including the dewatering discharge point and <br />sediment pond. The discharge ditch and flood irrigation ditch along the north side of the site were also <br />observed. The irrigation ditch was being used to irrigate the field north of the site. While the irrigation ditch <br />was running, the operator was performing dewatering pump maintenance for the pump in the north pit <br />resulting in groundwater in the pit and lower than normal water levels in the dewatering ditch. All monitoring <br />well locations, except MW -5 and MW -8 were observed. <br />The Division approved TR -07 on April 10, 2012. No problems were noted during the inspection.
