OBSERVATIONS <br />PERMIT #: PF.Id <br />INSPECTOR'S INITIALS: SJR <br />INSPECTION DATE: March 20, 2012 <br />This was a monitoring inspection of the Emerald Vista Estate; M- 1994 -006 operated by the Leone Sand and <br />Gravel. This site is located within the city limits of Trinidad, Colorado in Las Animas County. I, Stephanie Reigh <br />of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. Frank Leone and <br />Wayne Leone, of Leone Sand and Gravel accompanied me on the inspection. <br />This is a 9.4 acre 110c mining operation for sand and gravel. A mine identification sign was in place at the <br />entrance to the mine site. Permit boundaries were all clearly marked. There was no equipment on site. The <br />site is stable and no disturbance was observed outside of the approved permit boundaries. The highwall is <br />located in the northeastern part of the permit area. Mr. Leone indicated that they were going to initiate <br />reclamation this spring. Mr. Leone also had a question regarding building a house /shop on the property and <br />wanted to include that in the reclamation plan. He wanted to leave a hole where the foundation would be to <br />avoid having to fill /grade /topsoil just to open it up again. Based on the standards of the Divisions rules and <br />regulations, the entire permit area will need to be reclaimed as noted in the reclamation plan and the area will <br />need to be graded and topsoiled first. No depressions or holes shall be left for future building sites. <br />Topsoil is stockpiled at the southwestern extent of the current disturbance. In addition, little or no weeds <br />appeared to exist on this site. <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and <br />regulations set forth by the state and in the mining and reclamation plans for the pit. <br />Page 2 of 4 <br />
