September 28, 2011 C -1994 -082 /Yoast Min e SLB

Number of Partial Inspection this Fiscal Year: 5
Number of Complete Inspections this Fiscal Year: 1

All repair work in the Pond 012 slide repair area has been completed in accordance with the approved plan, with
the exception of seeding of the south slope. The area will be se eded next year, after a year of observation to
ensure that repaired slopes remain stable.
The operator regra ded a small settling crack on the east side of the YPM 5 -1 drainage. It did not appear to have
been seeded yet, and should be reseeded this fall.

Scarp regrading of the Neck Pit slide area was complete, but needed to be seeded. Seeding needs to be completed
this fall. The operator had constructed toe drains in the slide area, but they will need to b e reworked for positive
drainage. The current configuration retains three low spots where water will pool and cause the slide area to
resaturate. The operator will need to achieve a lower grade so that water runs from the slide into the Haul Road A
ditch . It might be necessary to construct a subsurface drain since excavation to the required depth would result in
either a deep, steep -sided trench or require removal of a large amount of material between the ditch and the slide.
Removal of the toe material to drain the low area would not be recommended as this material is currently
buttressing the slide.

ENFORCEMENT ACTIONS/COMPLIANCE

No enforcement actions were initiated as a result of this inspection, nor are any pending.
