Hickenlooper <br />Govemor <br />Mike King <br />Executive Director <br />Loretta Pineda <br />Director <br />RE: City of Greeley; 25 Avenue Site; File No. M- 2002 -020 <br />Second Incomplete 112 Construction Materials Reclamation Permit Amendment (AM -01) <br />Application Package <br />The Division of Reclamation, Mining and Safety (Division) has reviewed the content of the City <br />of Greeley amendment application for the 25 Avenue Site, File No. M- 2002 -020, and submits <br />the following comments. Preliminary review of the supplemental information received on March <br />23, 2012, determined the following items must be received before the Division can consider the <br />application submitted and technical review can begin: <br />Application Form: <br />1. The Division requires the Applicant /Operator name on the Certification page to match exactly <br />with the name listed on Item #1 of the application form and with the current Permittee listed <br />with the Division for the site. The Notary Public section on the Certification Page (page 8) of <br />the application form was not completed properly. Please submit a revised Certification Page <br />to the Division with the required corrections. A copy of an edited Certification Page of the <br />application form is attached for reference. Please note: the Notary Public section was <br />completed correctly in the original amendment permit submittal. <br />The amendment (AM -01) application will not be considered submitted until the information listed <br />above is received and found sufficient to begin our review. A decision date will be established 90 <br />days from the date of receipt of all of the requested information. Additionally, if you have <br />already published notice you will need to republish notice, after the Division considers the <br />application submitted. This notice must be published once a week for four (4) consecutive weeks, <br />starting within ten (10) days of the date your application is considered submitted. We will notify <br />you when you should initiate republication of your notice. The final date for receiving comments <br />is the 20th day after the fourth publication or the next regular business day. <br />You have sixty (60) days from the date of the first incompleteness letter to submit all necessary <br />documents the Office needs for the application to be considered filed.
