The Review Process <br />Summary <br />The Hamilton Mine permit was issued to Lillylands, Inc. on February 10, 1992. The approved permit <br />area encompassed a total 677 acres, of which 260 acres (in Mining Areas No. 1 and No. 2) were to be <br />disturbed. <br />Technical Revision No. 1 (TR -01), approved on June 29, 1992, reduced the disturbed acreage from 260 to <br />248 acres. <br />On August 4, 1992, Permit Revision No. 1 (PR -01) added 520 acres (Mining Area No. 3) to the permit, <br />which included 190 acres of additional disturbance. <br />The Hamilton Mine permit was transferred from Lillylands to Honeywood Coal Company under <br />Succession of Operator No. 1 (SO -01) on December 22, 1992. <br />The hydrologic monitoring plan was revised with Technical Revision No. 2 (TR -02) on May 17, 1993. <br />On October 7, 1993, the bond amount required for Mining Area No. 3 was reduced by $234,315 due to <br />the diminished area of disturbance. This reduction in liability was accomplished under Technical Revision <br />No. 3 (TR -03). <br />With Technical Revision No. 4 (TR 04), retention of the haul roads as permanent features was approved, <br />as requested by the landowner. TR -04 was approved on February 24, 1994. <br />On August 10, 1994, with SL-01, the Division approved Phase I bond release for the entire permit area, <br />exclusive of ponds and ditches. The bond amount was reduced to $152,774. <br />Technical Revision No. 5 (TR -05), approved November 2, 1994, incorporated permit modifications <br />requested in the Division's Midterm Review No. 1 (MT -01). <br />Technical Revision No. 6 (TR -06) revised the water monitoring plan on Nov. 14, 1995. <br />The Hamilton Mine permit was renewed for a second 5 -year term with Permit Renewal No. 1 (RN -01) in <br />February 1997. <br />Cessation of hydrologic monitoring was approved with Technical Revision No. 7 (Tit-07) on July 21, <br />1999. <br />Under Technical Revision No. 8 (TR 08), the permit area was reduced by 440 acres, to a total of 757 <br />acres. This was done to reflect the diminished area of mining that occurred at Mining Area No. 3. The <br />Disturbed Area was reduced to 111.2 acres, which accurately reflects the entire area disturbed by mining <br />operations. <br />Two additional successive five -year permit renewals were approved (RN -02 and RN -03), with effective <br />dates of February 27, 2002 and March 13, 2007, respectively. With RN -03, the Reclamation Cost <br />Estimate was updated, with the result that the required bond amount was increased to $447,281. <br />On October 13, 2009, Technical Revision No. 10 (TR -10) incorporated a soil loss demonstration into the <br />permit. <br />Technical Revision No. 9 (TR 09) approved the retention of Sediment Pond B as a permanent <br />impoundment on February 28, 2011. <br />Phase II bond release was granted for 86.8 acres with the Division's approval of SL -02 on April 14, 2011. <br />April 3, 2012 <br />RN4 <br />3 <br />
