February 22, 2012 C -1996 -084 /Lorencito Canyon Mine JHB <br /> <br /> <br />Number of Partial Inspection this Fiscal Year: 5 <br />Number of Complete Inspections this Fiscal Year: 2 <br /> <br /> Page 4 of 8 <br /> <br />Culv erts are numbered for this inspection as culverts encountered east to west along the access road. <br /> <br />Culverts: Culvert 1 -(ditch D97 to Ditch D98) Both inlet and outlet are slightly crushed. NECC needs to open <br />both inlet and outlet of this culvert <br /> <br />Culvert 2 -(36 inch culvert). This c ulvert appears to have a min i mum of 1 foot of material over the culvert. A <br />portion of the culvert has been pushed down i n the center. Flow does not appear impeded currently, but this <br />culvert needs to be opened up to assure ade quate flow volume is maintained. Outlet side of this culvert requires <br />clean out of sediment. <br /> <br />Cu lvert 3 -(48” concrete culvert) Outlet end of this culvert is approximately 1/3 full of sediment. Clean out of this <br />material is advised. <br /> <br />Culvert 4 - (24” dia.) This culvert has been cleaned out. Grading of the road has reduced the cover over this <br />culvert to 1 fo o t or less. NECC must assure that adequate cover is maintained over the culvert. <br /> <br />Culvert 5 Inlet of this culvert is fine. Outlet requires sediment c lean out. <br /> <br />five culverts under Pond 5 access road are all clear and functional. <br /> <br />OTHER (SPECIFY): <br /> <br />ROADS – Rule 4.03 Construction 4.03.1(3)/4.03.2(3) Drainage 4.03.1(4)/4.03.2(4) Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) Reclamat ion 4.03.1(7)/4.0 3.2(7): <br />The mine access road is also used by landowners as a ranch access road, and as a gas well access road. A third <br />party grades the road. The hole in the road identified in the January 2012 inspection report has been repaired. The <br />access road has been maintained in good condition. NECC needs to assure that traffic remains on the approved <br />access road and does not short -cut across reclaimed areas. <br /> <br />REVEGETATION – Rule 4.15 Vegetative Cover; Timing: <br />The vegetation throughout the min e sit remains mostly dormant. Crowns of some of the cool -season grasses are <br />showing some greening up due to moisture from recent precipitation events. Slopes exhibiting very thin <br />vegetation establishment and resulting rilling need to be evaluated for ril l repair, surface roughening and repaired <br />area seeding prior to this growing season.
