Gillette Sand and Gravel was <br />represented during the inspection by Chris Pyles. The Gillette Pit site entrance is about 1 /4 mile south of the <br />intersection of State Hwy 67 and County Road. <br />Inspection: <br />A permit sign was displayed at the site entrance (see Photo 1). The area of the site inspected is surrounded by <br />fence and access is controlled by gates. <br />The primary focus of this inspection was on the following six areas for which Mr. Pyles was requesting a <br />release (reference Attachment A): <br />TOTAL: <br />Current Stipulations: <br />None. <br />3.24 <br />OBSERVATIONS <br />* The Division will notify the Operator of released areas in a separate letter. <br />$ See recommendations below. <br />1.28 <br />File Review Notes: <br />• The current total permitted area is 32 acres. <br />• The previous inspection was performed on 11/19/09. No problems were noted. <br />• There is a $37,613.00 financial warranty (cash) for the site. <br />• Annual fees and reports are up to date through 2012. <br />• The post -mine land use is wildlife habitat. <br />PERMIT #: M- 1992 -009 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: March 9, 2012 <br />An erosion feature was observed near the south end of RA #7. This should be repaired before it becomes a <br />significant problem. <br />Recommendations: <br />The Division recommends the following tasks be completed by the Operator: <br />1. Area RA #7 — The Division acknowledges the less than ideal topsoil near the creek.
