WASTE NA (SC) EROSION /SEDIMENTATION - -- Y (RS) RECL PLAN /COMP -- Y <br />(AT) ACID OR TOXIC MATERIALS NA (OD) OFF -SITE DAMAGE Y (ST) STIPULATIONS NA <br />Y = Inspected and found in compliance / N = Not inspected / NA = Not applicable to this operation / PB = Problem cited / PV = Possible violation cited <br />OBSERVATIONS <br />PERMIT #: M- 2004 -025 <br />INSPECTOR'S INITIALS: THM <br />INSPECTION DATE: March 6, 2012 <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety (DRMS). Both Mr. and Mrs. Farnsworth accompanied the inspector during the <br />inspection. The North Fork Pit is a 9.97 acre gravel pit located approximately 1.3 miles west of Paonia, Colorado in Delta <br />County. <br />The mine identification sign and affected area boundary markers were not observed per the requirements of Rule 3.1.12. <br />The operator must post a sign at the entrance to the mine site which shall be clearly visible from the access road and <br />specify the following; the name of the operator, indicate that a reclamation permit for the operation has been issued by the <br />Colorado Mined Land Reclamation Board, and the permit number. The boundaries of the affected area must be marked <br />by monuments or other markers that are clearly visible and adequate to delineate such boundaries. The required <br />boundaries and sign must be placed and photo- documentation that proves the issue has been addressed must be submitted <br />to this office no later than April 12, 2012. <br />Evidence of recent mining and reclamation was observed during the inspection. The topsoil stockpiles were observed <br />adjacent to the eastern permit boundary and they appeared to be stable and quantity observed appeared to be adequate to <br />perform required reclamation.. <br />No noxious weeds were observed during the inspection. <br />The currently held bond amount of $22,212.00 is adequate at this time. <br />If there are any questions regarding this inspection report please contact me at (970) 241 -2042. <br />Page 2 of 4 <br />
