GENERAL REQUIREMENTS <br />I. Signatory Requirements <br />a) All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with the <br />following criteria: <br />b) Changes to authorization. If an authorization under paragraph a) of this section is no longer accurate because a <br />different individual or position has responsibility for the overall operation of the facility. a new authorization <br />satisfy mg the requirements of paragraph a) of this section must be submitted to the Division. prior to or together <br />ith any reports_ information. or applications to be signed by an authorized representative. <br />c) Certification. Monitoring <br />PART 1 <br />Permit - Page 16 <br />Permit No. COR- 030000 <br />1) In the case of corporations. by a principal executive officer of at least the level of vice- president or his or her <br />duly authorized representative. if such representative is responsible fir the overall operation of the facility from <br />which the discharge described in the form originates: <br />2) In the case ofa partnership. by a general partner; <br />3) In the case ofa sole proprietorship, by the proprietor; <br />4) In the case ofa municipal, state. or other public facility. by either a principal executive officer, ranking elected <br />official. or other duly authorized employee, if such representative is responsible for the overall operation of the <br />facility from which the discharge described in the form originates. <br />"1 certify under penalty of law that this document and all attachments were prepared under my direction or <br />supervision in accordance with a system designed to assure that qualified personnel properly gather and <br />evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or <br />those persons directly responsible for gathering the information, the information submitted is, to the best of my <br />knowledge and belief, true. accurate, and complete. I am aware that there are significant penalties for <br />submitting false information. including the possibility of fine and imprisonment for knowing violations... <br />a) The permitter shall retain copies of the SWMP and all reports required by this permit and records of all data used to <br />complete the application to he covered by this permit. for three years after expiration or inactivation ofpermit <br />co\ erage. <br />b) The permitter shall retain a copy of the SWMP required by this permit at the construction site from the date of <br />project initiation to the date of expiration or inactivation of permit coverage, unless another location, specified by <br />the permittee. is approved by the Division. <br />I he Di\ ision reser\ es the right to require sampling and testing, on a case-by-case basis (see Part I.D.I.e). for example to <br />implement the pro rsrons of a "I N1D1 (see Part 1.1) I 1 of the permit). Reporting procedures for any monitoring data <br />collected \ 111 he included in the notification by the Di\ ision of monitoring requirements <br />If monitoring is required. the lollowrng definitions apply: <br />a) The thirty (30) day average shall he determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive -day period. <br />b) A grab sample, for monitoring requirements, is a single "dip and take" sample. <br />
