to provide more guidance to operators with regard to inspection and maintenance of <br />signage. Copies of revised inspection procedure documentation and operator training <br />records were provided to confirm that the procedure had been implemented, as well as <br />photographic evidence showing that the illegible signage had been restored. In addition <br />to signage, procedures require employees not to eat, drink or smoke in the leaching, <br />processing or control room areas and to wash hands before eating and drinking. This <br />requirement is also reinforced during annual refresher training. <br />Eyewash/shower units are located throughout the ADR Plant where chemicals are <br />handled. The units are checked daily by the process operators during pre -shift safety <br />inspections and monthly during ADR Emergency Equipment inspections. With few <br />exceptions eyewash/ shower operations were recorded as operating correctly. Where <br />issues were identified during inspections, maintenance records indicate that they were <br />dealt with through CC &V's maintenance and repair program. Of approximately 20 <br />shower /eyewash stations at the ADR facility checked during the audit, three, although <br />still functioning in the event of an emergency, were observed to be in need of adjustment <br />(eyewash fountain too low, too high) or repair (flow in one eye piece significantly <br />obstructed). CC &V indicated the flow obstruction had occurred that day and was found <br />to be the result of a clogged filter. CC &V was asked to modify the inspection procedure <br />to include simple guidance criteria to help operators decide when eye -wash station flow <br />should be adjusted. The one station was repaired and the flow at the other two stations <br />was adjusted prior to the auditors leaving the site. Prior to submission of this report the <br />inspection procedure was modified and operators were trained in the use of the revised <br />procedure. <br />Only ABC type fire extinguishers are located where cyanide is handled. Fire <br />extinguishers are checked monthly during emergency equipment inspections. The units <br />were observed to be easily accessible and were clearly tagged. <br />First Aid procedures for cyanide are posted at strategic locations about the ADR facility. <br />There is also an emergency response and cyanide exposure procedure that provides <br />information on cyanide exposure symptoms and guidance on first aid treatment. Copies <br />of MSDS of Bulk Chemicals, including sodium cyanide are also provided in the <br />Emergency Response Plan (ERP). Master copies of MSDSs are maintained in the central <br />filing system. The electronic copy is accessible to all employees via the company <br />intranet or through Security. MSDS are in English, the language of the workforce. <br />CC &V has an incident reporting and investigation procedure that covers all operations, <br />including cyanide handling. The process is also used for reporting and investigating near <br />misses. Incidents are investigated by the supervisor and reviewed by management. <br />Records are maintained on JDE and hard copy by the Safety Department.
