PART <br />A. COVERAGE UNDER THIS PERMIT <br />1. Eli ibili <br />PART I <br />Page 4 <br />Permit No. COG - 603000 <br />To be considered eligible for authorization to discharge under the terms and conditions of this permit, the owner or operator of <br />any facility desiring to discharge subterranean structure (basement, foundation, footer drains, etc.) dewatering and /or well <br />development water with no evidence of contamination to ground and/or surface waters of the State must submit a complete <br />permit application form obtained from the Division. At least thirty days prior to the anticipated date of discharge, the application <br />shall be submitted to: <br />Colorado Department of Public Health and Environment <br />WQCD -P -B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246 -1530 <br />The specific application for this general permit with instructions is available as a hardcopy for pick -up, by calling (303) 692- <br />3500, or online at the Division's website: http• / /www cdphe state co us /wq/PernlitsUnit /Industrial /index.htmi . The Division <br />has thirty days after receipt of the above information to request additional data and/or deny the authorization for any particular <br />discharge. Upon receipt of additional information, the Division has an additional thirty days to issue or deny authorization for <br />the particular discharge. <br />If the Division determines that the operation does not fall under the authority of the general permit, then the information received <br />will be treated as application for an individual permit. In such case, discharge is not allowed until a permit is issued, which may <br />take 180 days. <br />This general permit will expire on December 31, 2012. The Division must evaluate this general permit at least once every <br />five years and must also recertify all existing permittees' authority to discharge under the general permit at such time. <br />Permittees desiring continued coverage under this general permit must re -apply 180 days prior to the expiration date of this <br />general permit. The Division will review all applications and determine on a case -by -case basis if permittees are eligible to <br />continue to operate under the terms of the general permit. An application for an individual permit will be required for any <br />point source discharge not reauthorized to discharge under the reissued general permit. <br />2. Application Requirements <br />The application referenced above will require the following information: <br />a. Name, address, and descriptive location of the facility (e.g. wells), including latitude and longitude; <br />b. Names and contact information for legal contact and principal in charge of the project or operation of the facility; <br />c. Description of the type of activity resulting in the discharge, including the anticipated duration of the activity and/or the <br />discharge, anticipated volume and rate of discharge, and the source of water, which is to be discharged; <br />d. Name of potential receiving State Waters, (including irrigation ditches, intermittent streams, dry drainage, and <br />groundwater); <br />e. Where pollutants are expected in the discharge, a description of the wastewater treatment system and /or recycle /reuse <br />utilized that will effectively and consistently meet all applicable effluent limitations (failure to provide satisfactory treatment <br />may result in immediate denial); <br />f. A topographic map showing the general geographical location of the facility, discharge locations, nearby landfills, mine <br />and /or mill tailings, and drinking water intakes; <br />g. A sketch of the facility and/or project showing all structures, discharge points, sampling locations, outfalls, and receiving <br />waters, as well as storage locations of any petroleum or chemicals on site; <br />h. A chemical analysis of the water to be discharged (only if requested by permit writer); <br />3. Certification Requirements <br />Under this general permit, facilities performing subterranean structure (basement, foundation, footer drains, etc.) <br />dewatering and /or well development water may be granted authorization to discharge process generated wastewater effluent <br />to ground and surface waters of the State. In order to be eligible for coverage under this general permit, the source water should <br />have no evidence of contamination. <br />An entity can be granted blanket coverage under this permit when testing of multiple wells or similar operations is being <br />performed in the same geographic area providing common permit terms and conditions are appropriate. The certification will <br />describe the physical boundaries authorized under that certification and any special conditions that may apply. <br />
