The revision was called complete for review on March 5, 2012. There are several adequacy <br />review issues that will need to be addressed prior to the Division's approval of the revision. <br />Section 2.03 Legal, Financial, Compliance and Related Information Revision <br />1.) In accordance with Rule 2.03.4 (2), the Operator indicated in the MR the address for Honeywood Coal <br />Company is 2120 Crestmoor Road, suite 3020, Nashville, TN 87215. However, the Division checked this <br />information with the Applicant Violator System (AVS) and it indicated the address for the Honeywood Coal <br />Company is 95 White Bridge Road, No. 404, Nashville, TN 87205. Please confirm which address is correct? If the <br />address provided in the minor revision is incorrect please send a revised page with the correct information. <br />Section 2.05.6(3) Water Monitoring Plan. <br />2.) The minor revision updated the language of the current approved monitoring plan. This was done as a result of <br />the Divisions preliminary review of the pending Phase III bond release (SL -03). The Division preliminary review <br />requested the Permittee to update the plan to include a full suite analysis for the bedrock wells. However, the <br />updated plan submitted only committed to measuring conductivity semi - annually (Monitoring Schedule #2). The <br />applicant will need to revise page 2.05 -35 and commit to conducting a fall full suite analyses for bedrock wells. <br />3.) Well completion information was requested back in July of 1998 and again in the Division's preliminary review <br />of SL -03 and was not provided. This information is critical to assessing groundwater quantity and quality impacts. <br />Well completion information is unavailable in the permit file other than a "typical" drawing for an alluvial well and a <br />bedrock well and the total depth of the wells listed next to the well location for some but not all wells. Please submit <br />well completion information. <br />4.) The applicant noted alluvial wells -4, 5, 6 &7 were destroyed in a local flood. In accordance with Rule 4.07.3 these <br />wells need to be properly abandoned. An abandonment report must be sent to the Division within 60 days of sealing <br />all wells.
