PART <br />A. COVERAGE UNDER T141S PERMIT <br />Authority to Discharge <br />Page 2 <br />Permit No. CUR - 010000 <br />Under this general permit, facilities engaged in light industrial activity as listed in Pan I.A.S are granted authorization <br />discharge stormwater associated with industrial activity into waters of the state of Colorado. <br />2. Application, Due Dates <br />a. Application Due Dates: At least thirty days prior to the anticipated date of discharge, the owner (or operator if the <br />owner does not operate the facility) of the facility shall submit an application as provided by the Division. <br />One original of the completed permit application foul shall be submitted to: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD -P -132 <br />4300 Cheny Creek Drive South <br />Denver, Colorado 80246 -1530 <br />b. Application Form: The application form requires, at a minimum, the following information: <br />1) Name and address of principal in charge of operation of the facility; <br />2) Site address and location; <br />3) Facility contact person, phone number and email address (if available); <br />4) Standard Industrial Classification (SiC) code(s); <br />5) List of other environmental permits currently held by the facility; <br />6) Facility description /industrial activities which take place at the site; <br />7) Receiving waters; and <br />8) Certification as to the completion of a Stormwater Management Plan (SWMP). <br />3. Permit Certification Procedures <br />If the general permit is applicable, then a certification will be developed and the applicant will be ratified under this general <br />permit. <br />Request for Additional Information: The Division shall have up to thirty days afler receipt of the above information <br />to request additional data and /or deny the authorization for any particular discharge. Upon receipt of additional <br />information, the Division shall have an additional fifteen days to issue or deny authorization for the particular discharge. <br />(Notification of denial shall be by letter, um cases where coverage under an alternate general permit or an individual <br />permit is required, instead of coverage tinder this general permit.) <br />Automatic Coverage: If the applicant does not receive a request for additional information or a notification of denial <br />from the Division dated within thirty days of the receipt of the application by the Division, authorization to discharge in <br />accordance with the conditions of this permit shall be deemed granted. <br />Individual Permit Required: If, after evaluation of the application (or additional information, such as the SWMP), it is <br />found that this general permit is not applicable to the operation, then the application will be processed as one for an <br />individual permit. The applicant will be notified of the Division's decision to deny certification under this general <br />permit. For an individual penuit, additional information may be requested, ano ISO days will be required to process the <br />application and issue the permit. Temporary coverage under this general permit may be allowed until the individual <br />permit goes into effect. <br />J <br />
