Under this general permit, facilities engaged in light industrial activity as listed in Pan I.A.S are granted authorization
discharge stormwater associated with industrial activity into waters of the state of Colorado.
2. Application, Due Dates
a. Application Due Dates: At least thirty days prior to the anticipated date of discharge, the owner (or operator if the
owner does not operate the facility) of the facility shall submit an application as provided by the Division.
One original of the completed permit application foul shall be submitted to:

b. Application Form: The application form requires, at a minimum, the following information:
1) Name and address of principal in charge of operation of the facility;
2) Site address and location;
3) Facility contact person, phone number and email address (if available);
4) Standard Industrial Classification (SiC) code(s);
5) List of other environmental permits currently held by the facility;
6) Facility description /industrial activities which take place at the site;
7) Receiving waters; and
8) Certification as to the completion of a Stormwater Management Plan (SWMP).
3. Permit Certification Procedures
If the general permit is applicable, then a certification will be developed and the applicant will be ratified under this general
permit.
Request for Additional Information: The Division shall have up to thirty days afler receipt of the above information
to request additional data and /or deny the authorization for any particular discharge. Upon receipt of additional
information, the Division shall have an additional fifteen days to issue or deny authorization for the particular discharge.
(Notification of denial shall be by letter, um cases where coverage under an alternate general permit or an individual
permit is required, instead of coverage tinder this general permit.)
Automatic Coverage: If the applicant does not receive a request for additional information or a notification of denial
from the Division dated within thirty days of the receipt of the application by the Division, authorization to discharge in
accordance with the conditions of this permit shall be deemed granted.
Individual Permit Required: If, after evaluation of the application (or additional information, such as the SWMP), it is
found that this general permit is not applicable to the operation, then the application will be processed as one for an
individual permit. The applicant will be notified of the Division's decision to deny certification under this general
permit. For an individual penuit, additional information may be requested, ano ISO days will be required to process the
application and issue the permit. Temporary coverage under this general permit may be allowed until the individual
permit goes into effect.
