Box 459 <br />Florence CO 81226 <br />Re: Southfield Mine, C1981 - 014, Subsidence Features <br />Dear Mr. Patterson: <br />On December 14, 2011 you accompanied me to six subsidence features located within the Southfield <br />Mine Permit. During the inspection I collected photographs and GPS coordinates of the subsidence <br />features. Please refer to the Division's December 14, 2011 inspection report for additional detail <br />regarding these features. <br />Correlation with mine plan map, Map 17, dated June 14, 1994, and inquiry of Inactive Mine Program <br />Supervisor,Bruce Stover, project manager on AML project 08123, the Division considers these six <br />subsidence features to be mining related. The features are located north of the Thompson Ranch <br />buildings. The Southfield permit section 2.05.6 provides predictions of potential subsidence impacts and <br />mitigative measures to be implemented. Although the observed subsidence features do not appear to <br />affect previously identified structures, the Division has concerns regarding safety of animals and humans <br />on the surface. The subsidence features consist of collapse holes of eight to twelve feet deep. Mitigation <br />of these features is necessary to assure animals and humans do not fall into these openings. Rule <br />2.05.6(6)(f)(iv) requires underground operations to provide a plan for mitigation of the effects of any <br />material damage or dimunation of value or foreseeable use of lands which may occur as a result of <br />subsidence. <br />Please provide a plan of action Energy Fuels plans to implement on the mitigation of the identified <br />subsidence features. Please notify the surface owner regarding proposed mitigation measures and <br />schedule. <br />Sincerely, <br />Janet H.
