July 19, 2010 <br />EMERGENCY RESPONSE PLAN - Initial Response Guide <br />PROCEDURE 1 -10 <br />APPENDIX 1 -10A <br />Page 1 of 1 <br />The initial Emergency Response Plan for the surface facilities at the Schwartzwalder Mine consists of two <br />phases. <br />PHASE 1 NOTIFICATION <br />Your PRIMARY RESPONSIBILITY as a First Responder in an emergency situation is the prompt <br />NOTIFICATION of the site EMERGENCY COORDINATOR or 911, if the Emergency Coordinator cannot be <br />contacted. <br />NOTIFICATION: <br />PHASE 2 OPERATIONS <br />(1) First Responder reports the following information to one other person immediately prior to starting any <br />direct operations for an accident, spill, or fire. <br />a) The location of the incident. <br />b) The nature of the incident. <br />c) The extent of injury, if applicable. <br />d) The type of material spilled, if known. <br />e) The physical extent of the spill area. <br />f) The First Responder =s intended course of action. <br />g) Method of maintaining communication. <br />(2) The first person contacted by the First Responder notifies the Emergency Coordinator. The <br />Emergency Coordinator for the Schwartzwalder Mine is: <br />RANDY WHICKER RSO PHONE: (970) 556 -1174 Pager: <br />Following initial notification site personnel are mobilized for operations. <br />OPERATIONS: <br />The Emergency Coordinator becomes the Incident Commander and activates the site Emergency <br />Alarm Horn to mobilize the site. <br />(2) All surface personnel assemble at their designated assembly point. <br />Cotter Personnel: Primary Assembly Point: Ion - Exchange Building <br />Back -up Assembly Points: Main Office Trailer <br />( 1) <br />Contractor and Visitor Personnel: Assembly Point: <br />Main Office Trailer <br />(3) The Incident Commander <br />X briefs the assembled response personnel. <br />X establishes a site command post and communications links on -site and off -site (911, Corporate <br />notification). <br />X reviews the situation and develops a plan of action for response operations. <br />X delegates support functions to other response personnel. <br />(4) The Response Operations Plan is implemented. <br />
