PERMIT #: M -1990 -112 <br />INSPECTOR'S INITIALS: TC1 <br />INSPECTION DATE: January 18, 2012 <br />OBSERVATIONS <br />This was a regular monitoring inspection conducted in order to check compliance of the operation with the <br />requirements of the Act, Rules, and the conditions of the permit. The operator, Lafarge West, Inc. was <br />represented during the inspection by Sean Frisch. The State Pit site entrance is located approximately three miles <br />south (on State Route 231/36`' Lane) of Vineland, Colorado in Pueblo County. <br />Inspection: <br />A permit sign was not visible from the entrance to the site (see Photo 1). A sign with the Permit No. was <br />observed on a pole behind the east scale house (see Photo 2). Both the east and west scale houses (see Photo 3) <br />obscure the view of the sign. The site was inactive at the time of the inspection and has not been mined for <br />some time. No stockpiles of material or topsoil were observed. Mr. Frisch stated some recent <br />grading/contouring had been completed in the south end of the site (see Photos 4 — 5). Erosion features were <br />observed on the some of the recently contoured slopes (in the "old pit "). All contoured slopes appeared to no <br />steeper than 3H:1 V. <br />Structures observed on the site in addition to the scale houses, include power lines (see Photo 3). Mr. Frisch <br />indicated the power lines and scale houses would be removed prior to requesting a reclamation release. <br />File Review Notes: <br />• The total permitted area is 1,046 acres: with a maximum of 755 affected acres, and a limit of 170 acres <br />disturbed at any one time. The disturbed area was measured using GIS (ArcMap) and found to be —126 <br />acres. <br />• The previous inspection was performed on 2/17/05. Two potential problems were noted and <br />subsequently abated: <br />1. Permit #, M- 1990 -112 needed to be added to the entrance sign, <br />2. A noxious weed management plan was required, primarily to control tamarisk. <br />• There is a $103,925 financial warranty (corporate surety) for the site. <br />• Annual fees and reports are up to date through 2011. Annual reports indicate no mining activity since <br />2005. Reclamation is ongoing. <br />• Previous annual reports indicate a majority of the site has been topsoiled and a small portion has been <br />seeded. No future mining has been anticipated since as back as the 2005 annual report. <br />Current Stipulations: <br />None. <br />Recommendations: <br />The Division recommends the following tasks be completed by Lafarge: <br />1. Pursuant to Rule 3.1.12, move the permit sign to the entrance gate such that it is visible from 36u' Lane, <br />2. The Division will assume the five -year reclamation period (C.R.S. 34- 32.5- 116(4)(q)(I)) began on the <br />date of this inspection, unless a letter from Lafarge is received by March 7, 2012 stating otherwise. If <br />mining is proposed in the future, the site should be placed in Temporary Cessation pursuant to C.R.S. <br />34- 32.5 -103 (11) (b). <br />3. Lafarge is required to repair the erosion features noted in Photo 4. <br />4. Lafarge will be required to remove, as agreed during the inspection, the two scale houses and the power <br />poles and lines prior to reclamation release. <br />5. Continue implementation of the weed control plan. A couple of tamarisk were observed, but they are <br />not currently of sufficient quantity to cite them as a potential problem. <br />Page 2 of 4 <br />
