Box 2526 <br />Longmont, CO 80501 <br />RE: Vasquez Quarry No. 2; DRMS File No. M- 1994 -121; Notice of Unresolved Problems <br />Dear Mr. Vasquez, <br />The above- mentioned site has had one or more problems noted that, according to Division of <br />Reclamation, Mining and Safety (Division) records have not yet been resolved. According to our records, <br />the following problem was to be resolved on or before September 30, 2011. Listed below is a description <br />and corrective action for the problem. Please resolve the corrective action and provide evidence of <br />such resolution on or before February 15, 2012. <br />PROBLEM: <br />• The current Mining Plan and Reclamation Plan need to be updated and clarified, including the <br />Mining Plan Map and the Reclamation Plan Map. <br />CORRECTIVE ACTION: <br />• The operator shall submit a Technical Revision, with the required $216 revision fee, to update <br />and clarify the current approved Mining Plan and Reclamation Plan to reflect existing and <br />proposed activities by the corrective action date. The Technical Revision shall also include <br />updated Mining Plan and Reclamation Plan Maps. The Mining Plan shall conform to the <br />requirements of Rule 6.3.3 and the Reclamation Plan shall conform to Rule 6.3.4. In addition, all <br />maps must conform to the requirements of Rule 6.2.1(2)(a) -(e) and Rule 6.3.5. Copies of the <br />above referenced Rules have been enclosed with this letter. <br />If the problem has already been remedied, please provide evidence of its completion.
