Optional Corrective Action 1 on the form should have referred to an Amendment, rather than a <br />Technical Revision. Although your subsequent submittal of a conversion application is consistent with <br />optional Corrective Action 2, a corrected "Staff Summary Form for Consent Agenda Items" form needs to <br />be signed so that the Board can consider the proper Corrective Actions. <br />Please sign and notarize the attached "Corrected Staff Summary Form for Consent Agenda Items" and <br />return the original signed copy to the Division.
