8. Stormwater vs. Process Water <br />Do you discharge any of the following process generated wastewaters from your facility, to state waters? <br />No XX Yes Product wash waters <br />XX No- Yes Maintenance /equipment wash waters <br />XX No_ Yes Transport waters (e.g., slurries) <br />XX No = Yes Scrubber waters (crushers or clarifiers) <br />1 No XX Yes Mine dewatering (groundwater and/or runoff from the mine) <br />NOTE: All plant process water will be run through series of temporary settling ponds before discharge. Ground water <br />pumped for pit dewatering will be pumped directly to on -site ditch. Site is presently unmined and no activity has occurred. <br />If you answered YES to any of these questions, continue on to Item 9. <br />If you answered NO to all of these questions, skip to Item 18. <br />9. Location map: A location map designating the facility property, discharge points and receiving waters shall be submitted. The <br />map shall be from a 71/4 or 15 minute USGS quad sheet, or a map of comparable scale. A north arrow shall be shown. The map <br />must be on paper 8 %S x 11 inches. <br />See Site Sketch for proposed discharge points. <br />10. Site Sketch: A legible general sketch of the site shall be submitted, showing appurtenant facilities (buildings, ponds, diversion <br />ditches, stockpiles, etc.), stream location, numbered discharge points, sampling and flow monitoring points. The map must be on <br />paper 8' 1/2 x 11 inches. The outfalls shall be labeled to correspond with the numbers listed in 15. <br />Site is presently unmined. Single discharge point presently planned per existing permit. <br />11. Site - specific conditions: <br />a) Does this facility have bulk storage of diesel fuel, gasoline, solvents, fertilizer, <br />or other hazardous materials on site? XX No 1 Yes <br />b) Is this operation located within one mile of a landfill, or any mine or mill tailings? <br />c) Does the dewatering area have or possibly have groundwater contamination, such as <br />Plumes from leaking underground storage tanks, etc.? <br />Note: Environmental Site Assessment for company ownership transition during 2004 indicated no such environmental <br />concerns. <br />If YES for any of these, please show location of the landfill, tailings or possible groundwater contamination on the location map in <br />item 8 or in the general sketch in item 10. Please explain the location, extent of contamination, and possible effect on the discharges <br />from this facility. <br />3 <br />XX No - Yes <br />XX No 1 Yes <br />
