Lupton Sand and Gravel Mine, Permit M- 1999 -120, Technical Revision Approval, Revision TR -6 <br />Dear Ms. Shults: <br />Upon receipt of the adequacy response letter from LG Everist dated December 23 2011, withdrawing the request to <br />revise the revegetation requirements for the Hill- Oakley phase, on December 27, 2011 the Division of Reclamation, <br />Mining and Safety approved the Technical Revision application submitted to the Division on December 14, 2011, <br />addressing the following: <br />Remove the abandoned gas and oil well from the final configuration of Hill - Oakley phase, and leave the Eber <br />property in permitted area. <br />The terms of the Technical Revision 6 approved by the Division are hereby incorporated into Permit M- 1999 -120. <br />All other conditions and requirements of Permit M- 1999 -120 remain in full force and effect. <br />If you have any questions, please contact me. <br />Sincerely, <br />C 4 <br />Eric C.
