OBSERVATIONS <br />PERMIT #: M- 1988 -052 <br />INSPECTOR'S INITIALS: CBM <br />INSPECTION DATE: November 18, 2011 <br />1) The inspection was conducted to verify the placement of the new permit boundary markers Present <br />during the inspection was Mr. Rick Hunt the operator. <br />2) When the site was inspected on July 28, 2011, the Division identified the operator was mining in the <br />wrong location. When the permit was approved in 1988, the original mining location was supposed to <br />be SE % of the SW1 /4 OF Section 28, T6S, R64W.Howver, mining occurred in the SW1 /4 of the SE1 /4 <br />Section 28, T6S,R64W. With the approval of the program supervisor, the operator was allowed to <br />submit an amendment application (only the five pages, with a new location and surveyed map were <br />submitted with the amendment application as approved by DRMS. The mining and reclamation <br />exhibits remain as was approved in the original application. covering the new area. The issue was cited <br />as a possible problem. The Division also raised the financial warranty for the current disturbance to <br />$18,642.19. <br />3) The required boundary markers were posted covering the current affected and permit area. The <br />amendment will be approved as soon as the Division receives the corrections on page three of the <br />amended application. <br />Page 2 of 4 <br />
