Exhibit M — Other Permits and Licenses <br />Paul Banks and Associates, LLC <br />Additional permits and approvals that may required for the Debeque Gravel Permit <br />Project are listed below. This list is intended to represent all possible permits and should <br />not be construed as a final determination of which permits will be required; a number of <br />permits, for example, in this case, a 404 permit from the U.S. Army Corps of Engineers, <br />may not be necessary given the findings of current investigations. <br />• Mesa County Land Use Approval (Conditional Use Permit) <br />• CDPS State Discharge Permit/Stormwater Permit <br />• Air Permits (APEN or other Construction Permit) <br />• Road Access and Maintenance Agreements - An Access Permit will be obtained, if <br />necessary, for access onto local and /or state roads. <br />• Substitute Water Supply Plan /Gravel Pit Well Permit <br />• State Historic Preservation Office Clearance — Please see the results of a search of <br />the Colorado Inventory of Cultural Resources in Attachment A. There are no historic <br />sites present within the proposed permit boundary. <br />• Spill Prevention Control and Countermeasure (SPCC) Plan — The applicant will <br />prepare and implement a Spill Prevention Control and Countermeasures Plan if, at <br />any time, at least 1,320 gallons of petroleum products will be stored on site. <br />Debeque Sand and Gravel Resource 11019 <br />Oldcastle Southwest Group, Inc. 23 November 2011 <br />
