The Applicant has stated a complete surface and groundwater monitoring program will be <br />submitted as part of a future permit revision. Please specify when the Applicant anticipates <br />submitting the revision addressing surface and groundwater monitoring. Also, indicate how <br />many quarters of water quality data have been collected to date. <br />3. The Applicant has stated two septic tanks /fields will be installed at the site. Please clarify if <br />there are any surface features associated with the septic systems which will need to be <br />reclaimed. <br />4. The Applicant has stated the waste rock from the development of the new Cross Mine <br />Decline Portal will be made available to Boulder County. The approved Mining Plan states <br />waste rock from future development will be utilized as stope -fill in the mine. Please clarify if <br />waste rock from development will still be utilized as stope -fill in the mine. <br />5. The Applicant has stated no waste rock will be disposed underground until all appropriate <br />agency approvals are obtained. Please specify the agency approvals which will be obtained in <br />order to dispose of waste rock in the mine workings. Also, please clarify if the disposal of <br />waste rock underground has occurred at the Cross Gold Mine. <br />6. The proposed Amendment calls for constructing a new office and dry room. Please state how <br />the existing office and dry room will be utilized in the future. <br />7. The amended area includes the Caribou Mine office and a dry room. Please explain how <br />these structures will be utilized as it relates to the Mining Plan. <br />8. The Applicant has stated only one of the shafts will be utilized for ventilation and as a <br />secondary escape -way. Please specify which shaft will be utilized for these purposes. <br />9. The revised permit boundary now encompasses the Idaho Tunnel Portal. Please describe how <br />the Idaho Tunnel Portal will be utilized as it relates to the Mining Plan. Also, please provide <br />the Division with a brief narrative of the history of the Idaho Tunnel Portal. <br />10. Please describe how the new Cross Mine Decline Portal and the existing Cross Mine Portal <br />will be utilized as it relates to the Mining Plan. <br />11. Please describe how the new Cross Mine Decline Portal will be safeguarded while the site is <br />active. <br />6.3.4 Exhibit D — Reclamation Plan <br />12. The approved Reclamation Plan states native trees will be planted during reclamation of the <br />site. The seed mix which was submitted with Amendment — 01 did not include trees. In <br />
