October 12, 2011 C- 1981- 044/Williams Fork Mines JRS <br />HYDROLOGIC BALANCE - Rule 4.05 Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6, Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />- The Mine No. 5 dewatering pump was operating at the time of the inspection. The water was being pumped into <br />pond 5P -6 and flowing into ponds 5P -7 and 5P -5. The water was discharging from Pond 5P -7 into the Williams <br />Fork River at a rate of approximately 400 GPM. The discharge was clear. The pond embankments for these <br />discharge ponds were well vegetated and stable at the time of the inspection. MCM has filled in the large burrow <br />in the north side of the pond 5P -7 embankment. Jerry indicated that they would return to further repair this area <br />with concrete, to ensure its stability. <br />- Pond 5A -P 1 was dry. The pond embankment was well vegetated and stable at the time of the inspection and no <br />erosional problems were noted. <br />- Ponds SH -P 1 and SH -P2 were both dry at the time of the inspection. The pond embankments were well <br />vegetated and stable and no erosional problems were noted. <br />- Ponds 9 -P 1, 9 -P4, 9 -P5 and 9 -P6 were all dry at the time of the inspection. All of the pond embankments were <br />well vegetated and stable and no erosional problems were noted. <br />-Ponds 5-PI, 5 -P2 and 5 -P3 were all dry at the time of the inspection. All of the embankments were well <br />vegetated and stable and no erosional problems were noted. <br />PROCESSING WASTE /COAL MINE WASTE PILES — Rule 4.10 and 4.11 Drainage Control; Surface <br />Stabilization; Placement: <br />- The reclaimed Mine No. 9 refuse pile remains stable. The area is showing good growth of perennial grasses <br />among the annual weeds. There was a large stand of thistle, including musk and Scotch thistle, on the outer edge <br />of the pile that was sprayed. It appears as though the spraying has been successful. MCM will need to continue to <br />inspect and spray the reclaimed refuse pile as necessary. <br />- The remaining refuse pile is currently idle, like the rest of the mine. The pile area is stable and no problems were <br />noted. <br />ROADS — Rule 4.03 Construction 4.03.1(3)/4.03.2(3) Drainage 4.03.1(4)/4.03.2(4) Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) Reclamation 4.03.1(7)/4.03.2(7): <br />- The mine entrance and haul road to the Mine 5 area was well maintained and stable. There were no problems <br />noted along this road. <br />- The haul road to the Mine No. 5A portal area was also stable and well maintained. There is still one location, <br />along the bridge /fill on the east side of the mine, just south of the 7 -North Angle Well facility, that has formed a <br />gully off of the haul road. This area is beginning to head -cut and is beginning to cut into the road itself (as <br />opposed to being only on the outslope of the road). This erosion was shown to Jerry during the inspection.
