The Division's files indicate the original permit area was 30.06 acres. The Division <br />released 8.31 acres during the previously submitted acreage reduction (AR02) leaving <br />21.75 acres in the current permit area. Item #3 on the application form indicates the <br />current permit acreage is 21.73 acres. Please submit a new complete copy of the first <br />page of the application with the correct permit acreage indicated for Item #3. With the <br />addition of 12 acres, the new total permit area should be 33.75 acres. <br />The acreage on the first page of the application form has been corrected to reflect <br />the 21.75 acres in the current permit area and 33.75 in the new total permit area. <br />Attached is a copy of this page. <br />2. An applicant must initial all the "Responsibilities as a Permitte" items listed on Page 5 <br />and 6 of the application form.
