Describe how drill holes will be plugged (refer to Rule 5.4 of the Rules for required <br />abandonment procedures): <br />The plugging and abandonment procedure for any drill hole or monitoring well where logging or <br />other information indicates that the top of the annulus cement placed outside the longstring casing <br />is below the ground surface will be as follows: <br />1. Determine the free point of the longstring casing using conventional methods. <br />2. Remove inner tubing(s) as necessary to allow access to the free point. <br />3. Cut and remove the longstring casing at or slightly above the free point. <br />4. Pump and place a cement - bentonite (bentonite not to exceed 5% dry weight) grout into the <br />remaining longstring casing and open hole above free point. Bring grout to surface and top -off <br />as needed. <br />5. Remove cellar ring and cut surface casings below existing grade. <br />6. Set well monument marker, if required, and regrade and reclaim surface. <br />If the longstring casing cannot be removed above the free point, then the casing will be plugged <br />with cement - bentonite grout and the annulus will be plugged as best possible using surface tremie <br />methods. <br />4 <br />
