Mr. Ronald J. Valentine, PE <br />Climax Molybdenum Company <br />February 17, 2004 <br />Page 3 <br />intake structure was then encased in concrete and a level platform for the building established. A <br />fiberglass, reinforced, cast -in- place, concrete structure was constructed above the intake structure. <br />The structure incorporated fiberglass reinforcement and used sulfate resistant concrete. A HDPE <br />baffle was installed around the intake and the gate, gate operator, and fiberglass stop logs installed. <br />A perimeter rail was also installed around the building deck. <br />FIELD TESTING AND OBSERVATION <br />Construction management and oversight was performed in house by Climax. Verification of <br />reinforcing bar installation, spacing and overlap, and the general structure dimensions were also <br />completed by Climax. Independent third -party firms were contracted to perform QA/QC testing of <br />critical materials. URS was contracted to collect samples and performing strength testing of the <br />grout and structural concrete. Visual inspection and dye penetration testing of the stainless steel <br />casing welds performed by AMEC Consultants. <br />On January 15, 2004, a representative from URS Corporation, Mr. Christopher Hatton visited the <br />Climax mine and observed the completed decant structure. During our visit, we reviewed project <br />files and the results of independent third -party QA/QC testing described above. The decant <br />operation and critical features such as the inlet weir horizontal plumbness was also observed. At the <br />time of our visit, the structure was functioning as designed. <br />Based on our observation and review of QA/QC and other construction documents, the construction <br />was completed in general accordance with the design drawings and approved specification.
