Engineering Construction Report for No. 7 Shaft Cap at Climax Mine by Ellis Environmental Engineering, Inc., February 2003 <br />4.0 Approach <br />Several individuals involved with the project were contacted to gain relevant facts <br />concerning the capping effort. The following people were interviewed by the author: <br />• Mr. Richard Gresham, P.E. - Former Owner and Chief Engineer with Ram <br />Engineering during the design and construction of the cap. <br />• Mr. Matt Kannegaard - Owner of Ram Specialists; worked on the shaft capping <br />job and provided the rough -draft of the As -Built drawing for the shaft. <br />• Mr. Stewart Teuscher - Engineer with Climax Molybdenum Company; original <br />engineering contact for contractors during the design phase of the project. <br />• Mr. Bryce Romig - Environmental Manager at Climax Mine during construction of <br />the No. 7 Shaft cap. <br />Climax Mine's file on No. 7 Shaft was also reviewed for pertinent details pertaining to <br />closure. The following documents were determined to be significant during my review: <br />• Four 8.5 x 14 drawings of various plan and section views of No. 7 Shaft prior to <br />capping. <br />• Ram Engineering letter to Stewart Teushcer, dated May 23, 1996 with two <br />alternatives for shaft plugging (includes sketches of two alternative designs). <br />• Ram Engineering Drawing 181 -A, dated June 30, 1996 with hand - written note "As- <br />Builts". <br />• Ram Engineering Quote for No. 7 Shaft Plug dated June 27, 1996 that lists <br />specifications for beams, re -bar, decking, and concrete. <br />• Several photographs taken by Mr. Bryce Romig during construction. <br />• Test results on three concrete cylinders obtained from the concrete slab dated <br />November 17, 2000 with an attached cover letter from Mr. Richard D. Reiter of Earth <br />Engineering Consultants, Inc. <br />The following additional information was obtained to assist in the engineering review: <br />• Test results from a composite sample of the fill around the shaft cap. Samples were <br />collected by the author on August 20, 2002 and tests were performed by ACZ <br />Laboratories in late August 2002. <br />• Photographs by the author taken during construction in August 1996. <br />• Another copy of Ram Engineering Drawing 181 -A with Mr. Matt Kannegaard's hand- <br />written notation about the steel I -beams used in construction. <br />A site visit to verify dimensions of the final cap was made by the author on August 20, <br />2002. Direct measurements of the length, width, and thickness were made. The locations <br />for the concrete test cylinders were verified, and soil samples from surrounding fill were <br />collected. <br />2 <br />
