PERMIT #: M- 2001 -060 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: August 18, 2011 <br />OBSERVATIONS <br />This was a partial inspection conducted as a follow up to the July 25, 2011 inspection. Jason <br />Musick, with the DRMS Coal program, was present. The purpose of this inspection was to <br />better characterize the imported material in order to determine whether or not it was coal mine <br />waste. Photographs are included at the end of this report in order to illustrate some of the <br />conditions observed during this inspection. <br />The stockpiled material located north of the pit during the July 25, 2011 inspection appeared to <br />have been moved approximately thirty feet to the west prior to this inspection. The amount of <br />material stockpiled in this area appeared unchanged though, and it is not believed that any of this <br />material had been removed from the site or placed into the pit following the last inspection. A <br />conveyor and screen had been placed near this stockpile. <br />As stated in the Division's previous inspection report, the imported material appeared to contain <br />a large coal component. There was also various debris intermixed with the material, such as <br />cinder blocks, rubber hoses, wood, and other trash. This stockpile appeared to contain <br />approximately 200 -300 yds.3 of material. <br />The stockpiled material located on the east side of the pit, which was to have been blended with <br />native dirt, according to the Operator, in order to reduce the coal component to less than five <br />percent, appeared undisturbed since the July 25, 2011 inspection. This pile appeared to have <br />been covered by a thin veneer of native material. Beneath the surface, the material was black <br />and appeared to contain a large coal component.
